Personal health information collected as part of psychotherapy may include contact information, intake information, appointment history, clinical notes, treatment information, communications, and billing records.

Only information reasonably necessary to provide services and operate the practice is collected.

Holly Owen Therapy uses Jane, a secure electronic practice-management system, to manage client files and other practice information.

Client records are maintained and protected in accordance with applicable privacy legislation and CRPO’s professional standards regarding confidentiality, recordkeeping, security, retention, and access.

Access to client information is limited to individuals who are authorized to access it for legitimate purposes.

Information shared during psychotherapy is confidential. Information will not be disclosed without your consent except where disclosure is permitted or required by law or professional obligations.

The nature and limits of confidentiality are discussed with clients as part of the informed consent process.

Please avoid sending highly sensitive or detailed personal health information through general email or website contact forms. Although reasonable safeguards are used, electronic communication can carry privacy and security risks.

Website forms and email are not appropriate for emergencies or urgent mental health concerns.

Credit card is the preferred method of payment.

Holly Owen Therapy does not provide direct billing to insurance companies. Following payment, clients receive a detailed receipt that can be submitted to their insurance provider for possible reimbursement.

Clients may request access to or correction of their personal health information, subject to applicable legal and professional requirements.

Clinical records are securely retained for the period required by CRPO and applicable Ontario legislation and are securely disposed of when the required retention period has ended.

Reasonable administrative, technical, and physical safeguards are used to protect personal health information from loss, theft, unauthorized access, disclosure, modification, or disposal.

If a privacy breach occurs, Holly Owen Therapy will respond in accordance with applicable privacy legislation and professional requirements.

Holly Owen is the designated Privacy Contact for Holly Owen Therapy and is responsible for overseeing the practice’s privacy practices and responding to questions, concerns, complaints, and requests related to personal and personal health information.
